Dry eye disease is defined by clinical criteria and international guidelines. From a systemic-medicine standpoint, these criteria remain the main reference; in addition, clinicians may consider whether ocular surface disease fits into a broader picture involving mucosae, metabolism, nervous system, hormones and lifestyle. 1. Gut symptoms and autoimmune disease

Conditions such as metabolic syndrome, diabetes, thyroid dysfunction and hormonal changes (for example peri- and post-menopause) are often associated with dry eye symptoms. Not all patients share these features, but when they are present, coordination between ophthalmologist, primary-care physician and other specialists is advisable so that ocular and systemic management stay aligned.

5. Environment, screens and lifestyle

Air conditioning, low humidity, smoke exposure, intensive screen use and reduced blinking can worsen dry eye symptoms. A systemic view does not replace drops or regulated therapies, but emphasises the value of practical measures (visual breaks, humidification, protection outdoors) and lifestyle choices that reduce overall irritative burden.

Dry eye diagnosis relies on eye examination, medical history and specific tests on tear film, inflammation and gland function. When general symptoms, known autoimmune diseases, metabolic disorders or hormonal issues are present, involving other specialists can be helpful. The aim is to integrate standard ophthalmic protocols with a broader view of the person, not to replace them.

Advanced therapies in current practice

Cyclosporine and topical immunomodulators

In moderate-to-severe dry eye, especially with marked inflammation, topical cyclosporine or other immunomodulators may be prescribed by the ophthalmologist to modulate chronic surface inflammation. These are regulated therapies, to be prescribed and monitored according to specialist indication. A systemic approach sees them as core treatments that can be complemented by work on comorbidities and lifestyle, never replaced.

Autologous serum and advanced tear substitutes

In selected, severe cases — markedly damaged ocular surfaces or dry eye refractory to conventional therapies — eye drops prepared from the patient's own blood (autologous serum or derivatives) may be considered. These preparations provide tear-like components and growth factors. They are specialist therapies, to be evaluated only after complete assessment and within centres following dedicated protocols.
